The ice pack stays cold and can penatrate cast material. Only downfall for me is all the Velcro, I bought extra ice pack inserts so I can use it on both my hands back to back. But with hand injurys it's very difficult to pull the Velcro apart
Pain in the but because you must wait for it to thaw some before you can manipulate around site. Back brace is soft immediately and easier to use.
